Request a Form/Survey setup by Rhsaas in your class-year and/or school directories utilizing Perl or someother cgi program format. Be sure to list the questions and possible answers. Be sure to list whether you choose text {fill in comments} or yes/no or selection choices of given answers. Send request to rhsaas-webmaster.
Form/Survey Services provided by various groups either free or by fee-based subscription exist on the internet. You can utilize your favorite search engine to locate one.
